Workshop/SDC Report Form for Branch/Centre Instructors

Please add the instructor team and their details that delivered the workshop/sdc e.g. Name, membership number, instructor grade and number.

The Lead Instructor needs to ensure they get all the student details e.g. Name, membership number and branch name.

John Smith - A000001 - Bingham 0378

The grade will only been awarded if the URN field is filled in or the candidate has purchased the pack direct (which will show on the BSAC database) - this can be generated by purchasing the correct course/workshop/assessment pack in the shop

If they are awarded a Pass/fail, given a grade or have done a refresher please select from drop down menu.
